  1. Describe the purpose of the Money Follows the Person demonstration
  2. List three ways that a transition specialist or peer can help individuals return to their communities of choice
  3. Identify the steps to take when an individual expresses an interest to return to community living from an institution
  4. Articulate one major principle of the Olmstead decision
